附件十目的
本表格用于收集机构工作人员对在本单位实习的学生社会工作者的意见，以供实习指导老师参考。表格应该由机构负责人或是机构代表填写。填写好的反馈表会给学生看，如果合适的话，还会进行机构员工、学生和实习老师的三方讨论。
